Class TypeSafetyTransformOrchestrator

Adds missing type annotations to a source file in three passes:

1. Parameters: sets unknown on untyped parameters.
2. Return types: infers the return type of untyped functions and methods, or falls back to unknown.
3. Properties: sets unknown on untyped class properties (same skip logic as the analysis rule).

Each pass checks for an existing type node first, so repeated runs are no-ops.
